CONFERENCE REGISTRATION

To register for the ERMR 2006 Conference, please fill out the registration form (you may type directly on either form):

• Registration form in PDF format
• Registration form in Microsoft Word format

When you submit the registration form, also submit one of the following payment forms:

• Credit Card Payment Form: PDF format; Microsoft Word format.
• Bank Transfer Payment Form: PDF format; Microsoft Word format.
• Cash Payment Form: PDF format; Microsoft Word format.

The registration and payment forms may be submitted by fax or mail to the Conference Secretariat.

The registration fees for the conference are as follows:
Before March 1, 2006: $380 USD/participant; $190 USD/student
After March 1, 2006: $480 USD/participant; $240 USD/student

The registration fee covers the proceedings, abstract book, coffee breaks, lunches, and conference banquet. The student registration fee does not include the conference proceedings.

Please note that the registration fee can be paid in USD at the registration desk in cash only during June 18-22, 2006.

The fee for spouse/companion attending luncheons and the banquet is $220.
The fee for spouse/companion attending the banquet is $70.
